If your insurance provider is not listed on this page, payment is due at the time the services are rendered, and you will be responsible for filing with your insurance company. In some cases, insurance companies do not require a provider to be on a pre-approved panel. On some plans, you may have "out of network" benefits that may be utilized. This will be handled on a case by case basis.

CANCELLATION POLICY
A minimum 24 hour notice is required for cancellation of appointments. Otherwise, you will be charged a $75 fee that will not be covered by your insurance plan.
